Paramedic – In-Home Sobriety Coach (Evening Shift)

Position Summary

We are launching a new service! We are seeking compassionate, dependable, and highly professional Paramedics to provide in-home sobriety support for individuals seeking income in home recovery for mild-moderate alcohol use disorder. This is a unique, patient-centered role that combines medical observation, encouragement, and crisis recognition during the highest-impact period to avoid relapse—the first week of sobriety. Working alongside a virtual supervising physician, you will provide evening support in patients' homes while physician-directed medication treatment (e.g., naltrexone and/or disulfiram) is initiated and managed.

This is not an emergency transport position . Instead, you'll serve as a trusted recovery coach, safety monitor, and a medical professional helping patients remain safe, comfortable, and committed to sobriety during the critical early stages of recovery. No talk therapy experience needed.

Typical Schedule: 5:00 PM – 11:00 PM

Responsibilities

• Visit patients in their homes during scheduled evening shifts.
• Provide supportive coaching, encouragement, accountability, and companionship during early recovery.
• Work with remote medical director to monitor patients for signs of alcohol withdrawal, or other medical concerns, and promptly communicate changes to the supervising physician.
• Reinforce physician-directed treatment plans and medication adherence.
• Educate patients and families regarding recovery strategies, relapse prevention, hydration, nutrition, and sleep hygiene.
• Assist patients in managing cravings using evidence-based behavioral techniques.
• Encourage engagement in recovery activities, support meetings, and healthy evening routines.
• Document patient interactions accurately within the electronic medical record.
• Escalate concerns appropriately, including arranging emergency evaluation when indicated.
• Maintain professionalism, patient confidentiality, and a calm, reassuring presence in the home.

Qualifications

Required

• Current EMT-Paramedic (EMT-P) license in good standing.
• Minimum one year of paramedic experience .
• Excellent interpersonal and communication skills.
• Comfortable working independently in patients' homes.
• Reliable transportation and willingness to travel locally.
• Strong documentation skills.
• Ability to remain calm, compassionate, and professional during emotionally challenging situations.
• While unlikely, comfortable administering IN/IM Lorazepam/Diazepam and recognizing opioid overdose when clinically indicated.

Preferred

• Experience working with patients with substance use disorders.
• Prior experience in behavioral health, addiction medicine, community paramedicine, emergency medicine, or critical care.
• BLS and ACLS certifications.

About Our Program

Our program combines physician-directed medication treatment—including medications such as naltrexone and disulfiram (Antabuse)—with in-home support during the first week of sobriety. Patients receive ongoing medical management while a dedicated sobriety coach provides structured evening support during the period when cravings and relapse risk are often greatest. Our goal is to help patients safely establish lasting recovery while remaining in the comfort of their own home.
