Addus Family Company seeks a Registered Nurse Admissions to complete in-home clinical assessments and coordinate safe, timely starts of care for home health and hospice patients. You will evaluate medical history and functional status, verify eligibility and payer requirements, develop initial care plans, and educate patients and families on services and safety. Partnering closely with physicians, intake, and field clinicians, you’ll ensure regulatory compliance, accurate documentation, and smooth transitions of care, supporting Addus’ mission to help vulnerable individuals remain safely at home.
