We're seeking an experienced and compassionate RN to join a growing Home Health team serving patients throughout the Austin area. This role offers the opportunity to provide one-on-one care, help patients maintain their independence, and become a trusted resource for patients and families in the comfort of their homes.

What You'll Do

• Perform comprehensive patient assessments and develop individualized care plans
• Provide skilled nursing care in patients' homes
• Educate patients and caregivers on disease management, medications, and treatment plans
• Monitor patient progress and communicate changes in condition
• Coordinate care with physicians, caregivers, therapists, and interdisciplinary team members
• Complete timely and accurate clinical documentation
• Support patients in maintaining their health, safety, and independence at home

Qualifications

• Active and unrestricted RN license
• Home Health experience preferred
• OASIS experience strongly preferred
• Current CPR certification
• Strong clinical assessment and critical thinking skills
• Excellent communication and patient education abilities
• Ability to work independently and manage a field-based schedule
• Valid driver's license and reliable transportation

Pay Details: $94,000.00 to $112,000.00 per year
