Rehousing Case Manager - Individual Homeless Services

Pay Range $24 to $27 per hour

Position Summary

The Rehousing Case Manager will be responsible for providing comprehensive, person-centered case management services to residents of a 19-bed Permanent Supportive Housing (PSH) program. The primary goal is to help individuals maintain stable housing, improve overall health and well-being, increase self-sufficiency, and connect with community resources. This Case Manager works collaboratively with residents, property management, healthcare providers, behavioral health professionals, and community partners to support long-term housing stability.

Essential Functions and Responsibilities

• Conduct intake assessments and develop individualized service plans for all residents
• Maintain an active caseload of up to 19 residents
• Meet regularly with residents to monitor progress and update service plans
• Provide crisis intervention, conflict resolution, and supportive counseling
• Assist residents in obtaining and maintaining public benefits and entitlements
• Coordinate referrals to medical, behavioral health, employment, education, and community resources
• Support residents in developing independent living and tenancy skills
• Advocate for residents and coordinate services with community providers
• Promote housing stability through wellness checks and early intervention
• Maintain accurate, timely, and confidential case records and documentation
• Complete assessments, progress notes, reports, and service plans in compliance with program requirements
• Monitor resident progress toward individualized goals and outcomes
• Participate in case conferences, staff meetings, and required trainings
• Coordinate resident move-ins, move-outs, and housing transitions
• Ensure compliance with HUD, CoC, state, and local regulations
• Participate in after-hours and on-call coverage as assigned
• Complete annual lease renewals and collect, track, and submit monthly rent payments
• Maintain the cleanliness, safety, and overall condition of the building
• Conduct weekly unit inspections to ensure resident wellness, safety, and lease compliance
• On-call duties as assigned

Qualifications

• High School Diploma or equivalent education
• Minimum of 3-5 years’ experience working with low-income individuals or families
• Minimum of one-year assessment, advocacy and case management work in a Human Service field
• Knowledge and experience working with low income, culturally diverse, and at-risk families and individuals
• Demonstrated knowledge of community resources available to low-income individuals and families
• Excellent customer service, organizational, oral and written communication skills
• Ability to manage multiple priorities and work in a fast-paced environment
• Proficient computer skills required in basic computer software and data collection systems
• Excellent crisis management skills
• Ability to maintain accurate records and confidentiality of information.
• Excellent attention to detail

Preferred Qualifications

• Bachelor’s Degree, preferably in a Human Services related field. Significant case management experience may substitute for degree
• Bilingual (English/Spanish, English/Khmer, English/Portuguese)
