Physical Therapist / Field Clinician

Location: Baltimore County & Baltimore City, Maryland

Employment Type: Full-Time

Travel Required: Yes – Community activities, office meetings, and patient home visits

Position Summary

We are seeking a dedicated and compassionate Physical Therapist / Field Clinician to provide home health and community-based physical therapy services throughout Baltimore County and Baltimore City. This role is responsible for evaluating patients, developing and implementing treatment plans, coordinating care with interdisciplinary teams, and maintaining accurate clinical documentation in accordance with physician orders, agency policies, and regulatory requirements.

Essential Duties & Responsibilities

Patient Assessment & Care Planning

• Conduct initial patient assessments when physical therapy is the sole ordered service.
• Determine patient eligibility for services, including Medicare homebound status when applicable.
• Complete comprehensive Start of Care (SOC) assessments within required timeframes.
• Collaborate with physicians and healthcare providers to assess functional status and develop individualized plans of care.
• Evaluate and re-evaluate patient rehabilitation needs at required intervals, including every 30 days or as clinically indicated.
• Utilize standardized assessment tools to monitor progress and treatment effectiveness.

Physical Therapy Treatment

• Develop and implement treatment plans designed to relieve pain, restore function, improve mobility, and maximize patient independence.
• Administer therapeutic exercises, modalities, and interventions according to physician orders and established care plans.
• Monitor patient response to treatment and communicate changes in condition to physicians, clinical managers, and care team members.
• Initiate preventive, diagnostic, and rehabilitative interventions as appropriate.

Patient & Caregiver Education

• Educate patients and families regarding:
• Disease management
• Home safety
• Fall prevention
• Infection control
• Mobility techniques
• Use of assistive devices
• Prosthetics and orthotics
• Therapeutic interventions and exercises
• Assess barriers to learning and provide individualized education tailored to patient needs.
• Utilize agency-approved educational tools and resources to support patient understanding and care planning.

Care Management & Coordination

• Manage patient episodes of care from Start of Care through discharge.
• Develop, implement, and monitor individualized care plans.
• Coordinate services with physicians, nurses, therapists, aides, and other interdisciplinary team members.
• Participate in patient care conferences and interdisciplinary meetings.
• Support continuity of care, recertification decisions, and discharge planning.
• Communicate plan-of-care changes to appropriate team members and caregivers.

Medication Review & Safety

• Perform medication reconciliation and review of all prescription and non-prescription medications.
• Identify and address:
• Adverse drug reactions
• Significant side effects
• Drug interactions
• Duplicate therapies
• Medication non-compliance
• Ineffective treatment regimens
• Provide medication education to patients, caregivers, and families.

Documentation & Compliance

• Complete clinical documentation accurately and in real time whenever possible.
• Submit required documentation within 24 hours when same-day completion is not feasible.
• Maintain complete, compliant, and timely patient records.
• Ensure adherence to agency policies, Medicare requirements, and regulatory standards.
• Participate in quality improvement initiatives and agency compliance programs.

Supervision & Leadership

• Supervise and educate home health aides as appropriate.
• Assess ongoing patient needs for supportive services and revise plans of care accordingly.
• Monitor and evaluate Physical Therapist Assistant (PTA) performance and patient outcomes.
• Conduct required PTA supervision in accordance with Maryland regulations and agency standards.
• Collaborate with clinical leadership regarding patient care coordination and discharge planning.

Professional Development

• Participate in required agency training, in-services, and continuing education programs.
• Stay current with industry standards, best practices, and regulatory requirements.
• Contribute to agency initiatives and support overall team success.

Qualifications

Required

• Doctor of Physical Therapy (DPT) degree from an accredited Physical Therapy program.
• Current Physical Therapist license in the State of Maryland.
• Valid driver's license.
• Current automobile liability insurance meeting company requirements.
• Ability to travel throughout Baltimore County and Baltimore City for patient visits and agency-related activities.
• Graduate of a physical therapy education program approved by:
• The Commission on Accreditation in Physical Therapy Education (CAPTE), or
• A successor organization recognized by CAPTE, or
• An international program determined to be substantially equivalent through a credential evaluation organization approved by the American Physical Therapy Association (APTA).
