About the Role:
As a Social Service Coordinator, you will play a vital role in providing emotional, social, and psychological support to our patients and their families. You will work closely with our interdisciplinary team to develop and implement individualized care plans that cater to each patient's unique needs and goals. Your primary responsibilities will include:
• Assessing patient needs and identifying opportunities for social and emotional growth
• Developing and implementing personalized care plans in collaboration with our healthcare team
• Providing emotional support to patients and their families through counseling and education
• Facilitating group activities and therapies to promote socialization and stimulation
• Maintaining accurate records of patient progress and care plans
Responsibilities:
• Coordinate with healthcare professionals to develop and implement individualized care plans
• Conduct social and emotional assessments to identify patient needs and opportunities for growth
• Provide emotional support and counseling to patients and their families
• Facilitate group activities and therapies to promote socialization and stimulation
• Maintain accurate records of patient progress and care plans
• Collaborate with other departments to ensure seamless communication and coordination
