Clinical Pharmacist- Specialty Pharmacy and Ambulatory Care

The University of Iowa Healthcare Department of Pharmaceutical Care invites full‑time Clinical Pharmacists to provide pharmaceutical care within the UI Specialty Pharmacy Program and the UI Healthcare Ambulatory Care Pharmacies. Services are delivered via face‑to‑face, mail order, and telehealth. Adequate training for Specialty Pharmacy and Ambulatory Care Services is provided. The pharmacist will rotate between the offsite central fill pharmacy and office locations in Iowa City, supporting both adult and pediatric ambulatory care operations on the UI Health Care Medical Campus. Sign‑on bonus up to $10,000 may be offered to eligible candidates. Position Duties Provides Therapeutic Drug Monitoring (TDM) Completes a comprehensive patient assessment, including medications, medical history, and clinical context. Evaluates prescription orders for: Indication, dose, route, drug interactions, organ‑function adjustments, incompatibilities, side effects, misuse risks, cost‑effective therapy, and duplicate therapy. Conducts pharmacokinetic assessment by clinical evaluation and prepares dosing recommendations. Communicates with prescribers about medication needs and updates patient profiles as required. Provides drug information to other healthcare professionals and confirms patient drug allergies. Participates in collecting medication histories and evaluates patient compliance, addressing potential non‑compliance issues. Coordinates financial resolutions with department counselors. Documents All Patient Care Activities Records patient drug allergies, vital signs, medication histories, counseling, prescriber and provider interventions, patient interactions, dosing recommendations, and drug use evaluation program activities. Coordinates Patient Communications and Interactions Educates patients on medication history, allergy history, teaching classes, counseling, and continuity of care with local providers. Facilitates Operational Initiatives Addresses medication system problems such as unclear orders, cost‑effective therapy, and non‑formulary prescriptions. Implements Medication Use Evaluation Program initiatives, including prescriber education. Serves on interdepartmental committees, task forces, and teams. Facilitates monthly clinic audits as directed. Provides Educational Services Precepts pharmacy students and residents, and presents in‑service education to staff nurses, physicians, and other team members. Participates in departmental, college, and state or national educational programs. Leads patient education classes and mentors other pharmacists. Facilitates Transitions of Care Assists with bedside delivery of discharge medications and provides medication education at the point of care to expedite discharge. Participates in Research Supports Investigational Drug Service prescriptions and orders; may engage in developmental, evaluative, or clinical research. Encouraged to present or publish findings. Maintains compliance with all competencies and educational requirements. Qualifications Education Requirements PharmD (Doctor of Pharmacy) degree or Bachelor of Science in Pharmacy. Experience Requirements At least one year of applicable pharmacy experience; student rotations may be considered. Immunization certification. Current licensure by the Iowa Board of Pharmacy is required (or eligibility for licensure). Excellent written and verbal communication skills. Professional experience working with diverse individuals. Desired Qualifications Completion of an ASHP‑accredited PGY1 residency. Health system pharmacy experience. Specialty pharmacy experience. Clinic order experience. Non‑sterile compounding experience. Board certification in ambulatory care, pharmacotherapy, oncology, specialty pharmacy, or other relevant area.
