Overview Clinical Liaison VIPC, Hospice Location: Glendale, CA (office-location) Position: Clinical Liaison VIPC, Hospice Position Type: Part-Time Remote/Virtual Position: No Coverage Area: Huntington Hospital / field-based compliance visits when there aren't Huntington Hospital patients Find Your Passion and Purpose as a Hospice Clinical Liaison VIPC Salary: $55 - $60 / hour Schedule: Every Other Weekend shift (20 hours) 8am-6:30pm Offer Based On Years of Experience What You Need to Know Key Reponsibilities: Build and maintain trusted relationships with physicians, hospital staff, and care teams through daily inperson collaboration and communication. Serve as the primary clinical resource for hospice education, eligibility, and appropriateness. Conduct patient assessments and coordinate the admission process. Partner with Admissions, Clinical, and Care Teams for smooth transitions. Advocate for patients and families during goalsofcare conversations. Participate in interdisciplinary planning, training, and outreach. Uphold ethics, quality, and continuous learning. Meet expectations for response times, referral coordination, and communication. Be the Best Inpatient Hospice Clinical Liaison Registered Nurse RN You Can Be If you meet these qualifications, we want to meet you!
