Prava Therapy is looking for a Program Director — Physical Therapist or Occupational Therapist to lead our therapy program at The Kensington Sierra Madre.

The Kensington Sierra Madre is an enhanced assisted living and memory care community where residents receive a high level of clinical support while remaining in a setting designed to feel like home.

That's where you come in.

As Program Director, you will be the clinical and community leader for Prava's therapy program at The Kensington Sierra Madre.

You will maintain your own clinical practice while leading the day-to-day therapy program and helping your team deliver exceptional resident care.

You will build relationships with residents, families, nursing, leadership, caregivers, wellness, and life enrichment.

You will know the residents.

You will know the community.

You will know your team.

And you will understand how all three come together to create a great therapy program.

• Lead the day-to-day clinical presence of the Prava therapy program within the community.

• Maintain an active clinical caseload while serving as a resource and mentor to the therapy team.

• Establish a culture of clinical excellence, accountability, collaboration, and resident-centered care.

• Orient and integrate new clinicians into the community, its residents, culture, routines, and expectations.

• Coach and mentor therapists in clinical reasoning, documentation, communication, and functional outcomes.

• Build strong relationships with community leadership, nursing, care partners, residents, and families.

• Participate in interdisciplinary conversations and help identify residents who may benefit from therapy.

• Ensure therapy goals are connected to what residents actually want and need to do in their daily lives.

• Help clinicians think beyond impairments and minutes toward function, participation, independence, and quality of life.

• Support residents with a wide range of needs, including mobility limitations, falls, neurologic conditions, orthopedic recovery, cognitive impairment, dementia, and changes in functional status.

• Partner with caregivers and community staff to reinforce strategies that help residents function safely and confidently throughout the day.

• Monitor clinical quality, documentation, outcomes, scheduling, caseload trends, and other key program indicators.

• Identify opportunities to strengthen the therapy program and work with Prava leadership to turn those opportunities into action.

• Create visibility for therapy throughout the community—not just inside the therapy space.

• Represent Prava as a clinical partner , not an outside vendor.

You don't have to be the loudest person in the room.

You understand that excellent therapy isn't about how many exercises you can prescribe.

It's about understanding the person in front of you.

What do they value?

What are they trying to get back to?

What matters to their family?

What is getting in their way?

And what can we change—in the person, the environment, the routine, or the support around them—to help them succeed?

You believe:

Every intervention should have a functional purpose.

The environment should be adapted before the resident is blamed for not adapting to it.

Residents should be appropriately challenged—not underestimated.

Outcomes matter more than minutes.

Caregiver education is part of treatment.

Dignity and autonomy are clinical priorities.

And you believe the best therapy programs are built through relationships.

That's one of the biggest differences in this role.

You'll see the same residents in the dining room, the hallway, an activity, or an exercise class.

You'll get to know their families.

You'll collaborate with the people caring for them every day.

You'll see what happens between therapy sessions.

And you'll have the opportunity to influence those moments.

That means your impact doesn't stop when the treatment session ends.

You become part of the community.

You bring clinical expertise.
