RN Case Manager

About This Role The RN Case Manager is responsible for ensuring high-value patient care that is coordinated and efficient, collaborating with the healthcare team to achieve specific patient outcomes and optimize resource use. Highlights Location: Westerly, RI What You’ll Do Coordinate and implement the plan of care as part of the interdisciplinary health care team. Establish a system for linking inpatient care with outpatient services and case management. Optimize hospital systems impacting quality and length of stay. Ensure patients meet clinical needs for admission, treatment, and discharge. Assist clinicians in documenting appropriateness of admissions and continued stays. Develop and implement appropriate discharge plans with healthcare team members. Contact outside agencies and initiate necessary referrals. Advocate for patients and families, linking them with appropriate resources. Requirements Education: Minimum of a Baccalaureate degree in clinically related field. R.N. required. Experience: Minimum of three (3) years of relevant clinical experience. Licenses & Certifications: RN Licensure in Rhode Island required. Additional RN licensure in CT preferred. #J-18808-Ljbffr
