- Assess patients in inpatient or outpatient settings and devise personalized treatment plans to meet their unique needs
- Implement focused physical therapy interventions aimed at enhancing mobility, strength, balance, and overall functionality
- Collaborate effectively with an interdisciplinary team, including nurses, doctors, and rehabilitation specialists, to achieve optimal patient outcomes
- Guide patients and their families through therapeutic objectives, discharge strategies, and home exercises
- Maintain accurate documentation of evaluations, treatments, and patient progress in line with facility and legal guidelines
- Engage in care team discussions and interdisciplinary meetings to promote coordinated care efforts
- Doctorate in Physical Therapy (DPT) or equivalent qualification from a recognized program
- State licensure as a Physical Therapist
- CPR certification is preferred
- Additional specialized certifications (e.g., neurological, geriatric) are advantageous but not mandatory
- Prior experience in physical therapy is highly preferred
- Capability to treat patients with intricate medical histories and mobility challenges
- Excellent communication and collaboration skills
- Proficient in using digital patient management and clinical documentation systems
