Community Support Consultant

Lutheran Social Services is seeking a Community Support Consultant to provide community-based support, service coordination, and advocacy for individuals and families facing behavioral health, housing, and social challenges. In this mission-driven role, you will conduct strengths-based assessments, develop individualized service plans, and connect clients to vital community resources. You will collaborate with interdisciplinary teams, document services, and support crisis and safety planning while promoting dignity, inclusion, and social justice in alignment with our faith-based, client-centered values.
