RN Homecare Manager

GEORGIA COMMUNITY SUPPORT AND SOLUT

This position oversees the clinical management and supervises the Home Health Aide staff. This is a leadership position, responsible for compliance with all federal, state, local regulations and policies and procedures pertaining to home health services. This position will perform duties described by the agency scope of practice for a licensed RN in the home, will perform patient admission and treatment visits and may function as the "primary nurse" for some patients. This position's assigned primary responsibility for planning and implementing the patient's care.

POSITION REQUIREMENTS:

Registered Nurse licensed in the State of Connecticut, with a minimum of two (2) years' nursing experience at least one (1) year of which must be in the provision of home health care; must have exceptional organizational and documentation skills with ability to make independent nursing decisions and demonstrate critical thinking skills. Supervisory experience a plus.


SPECIFIC SKILLS:
  • Ability to read, speak and understand the English language clearly
  • Strong interpersonal, organizational, and communication skills in person and via telephone
  • Must be well organized
  • Ability to make decisions with minimal supervision
  • Knowledge of medical terminology
  • Must be proficient in maintaining files and operating office equipment
  • Ability to maintain good rapport with staff, families, physicians and residents
  • Ability to work calmly and efficiently under pressure
  • Ability to relate to elderly in a dignified manner
  • Microsoft Office suite knowledge
  • Matrix Care familiarity preferred
ESSENTIAL FUNCTIONS
  1. Assess the health care needs of patients and function as primary nurse, as assigned, or work with the physicians and third party care agencies/resources, where appropriate, in developing and implementing a comprehensive plan of care/plan of treatment consistent with professional standards and agency policies.
  2. Identify community resources and assist patients/caregivers in accessing services, supplies, medications and equipment needed for continuity of care.
  3. Conduct, attend and participate in case conferences, team meetings, in-service education and staff meetings as held within the agency.
  4. Monitor agency caseload and ensure there are adequate staffing numbers for Home Health Aides to meet agency needs and growth. Delegate responsibility for twenty-four (24) hour nursing service and Homemaker-Home Health Aide services.
  5. Maintain professional licensure and other requirements (i.e. CEU credits) as required by all states in which the RN will practice.
  6. Demonstrate compliance with all agency policies and procedures, including patient care practice, clinical record documentation and educational commitments.
  7. Report patient progress to case managers, physicians and other team members as appropriate. Documents coordination of care and communication with other disciplines.
  8. Update the Coordinator/Scheduler of HHA's with changes to care plan or agency policy or procedures.
  9. Participate in discharge planning, discharge teaching and care transition coordination with other departments.
  10. Participate in on-call coverage, weekend coverage and holiday coverage as needed.
  11. Provide on-going supervision and clinical training to staff through consultation, instruction, orientation and in-service education.
  12. Conduct field visits, scheduled and unscheduled, to observe staff perform their assigned tasks.
  13. Responsible for the annual evaluation of all HHA staff and ensuring annual medical requirements are met, including PPD's and physicals.
  14. Active involvement in Quality Performance Improvement Program (QAPI), safety committee, risk management, and emergency preparedness
  15. Assist in developing the annual department budget with administration and finance
