Travel Social Worker Opportunity
Job Title: Social Worker

Contract Length: 12 weeks
Start Date: 08/24/2026
Job Description:
This field position involves providing social work care for Hospice and Home Health patients within their home setting. The role includes participating in weekly case conference processes and may require traveling using a personal or potentially assigned fleet vehicle.
Responsibilities:
- Providing psychosocial support and hospice services, including patient/family needs assessment, resource finding, and bereavement support
- Collaborating with the Interdisciplinary Group (IDG) through the IDG process
- Meeting home health and hospice criteria and regulatory requirements
- Managing personal schedules to typically see up to 5-6 patients per 8-hour day
Requirements:
- Minimum of 1.5 years of experience, including Home Health/Hospice
- BLS certification through the AHA
- SD Licensure for Social Work required; CSW license eligibility required
- MSW - Master's Degree in Social Work
- Valid Driver's License and Vehicle Insurance
