In-Home Traveler Physician Full Time - State of New Jersey and Surrounding Markets

About the Role Signify Health, part of CVS Health®, is seeking a Full‑time Physician to provide In‑Home Health Evaluations across a defined coverage area. Travel up to 100% of the time, including overnight stays and flights. Holding multiple state licenses is required. Key Responsibilities Conduct 45‑minute home visits for medication and medical history review, physical evaluation, and diagnostic tests (e.g., diabetic eye exams, spirometry). Use an iPad and clinical workflow; not required to prescribe medicine, order lab tests or alter treatment regimens. Deliver diagnostic and preventive services such as spirometry, DEE, bone density screening. Travel locally and within licensed states, and conduct virtual visits as needed. Build trusted relationships and connect patients to appropriate care. Work Environment Primary workplace is patient's home; may involve pets, secondhand smoke, and other allergens. Qualifications MD or DO with active, unrestricted license(s) in coverage area(s). Ability to obtain and maintain multi‑state licenses as required. Knowledge of New Jersey coverage area counties listed. Compensation & Benefits Pay range: $145.00 – $145.00 per evaluation (base). Additional pay per completed diagnostic and preventive services (DPS). Weekly hours: 40. Full‑time position.
