Lightways Hospice and Serious Illness Care
Lightways Hospice and Serious Illness Care is seeking an experienced RN Field Case Manager to provide skilled nursing assessments and hospice care to patients in home and community settings in Joliet and surrounding areas. The role requires an active RN license, graduation from an accredited nursing program, and a minimum of 2 years nursing experience, with preference for home health or hospice.
