Description

- Conduct thorough evaluations for patients in both inpatient and outpatient settings, tailoring individualized treatment plans to meet diverse needs.

- Implement physical therapy interventions with a keen focus on enhancing mobility, strength, balance, and overall function.

- Work collaboratively with nurses, physicians, case managers, and other rehabilitation professionals to drive optimal outcomes for patients.

- Educate patients and their families regarding therapeutic objectives, discharge planning, and home exercise protocols.

- Keep meticulous records of evaluations, treatments, and patient progress in accordance with facility and regulatory standards.

- Engage in interdisciplinary team meetings and care team huddles to ensure synchronized care delivery.

Education

Doctorate in Physical Therapy (DPT) or an equivalent qualification from an accredited institution.

Certifications

State licensure as a Licensed Physical Therapist.

CPR certification is preferred but not mandatory.

Specialized certifications in areas such as neurology or geriatrics are advantageous but not essential.

Skills

Prior experience is highly preferred; a demonstrated ability to address complex medical and mobility challenges.

Strong interpersonal skills and a capacity for effective teamwork.

Proficiency in using electronic medical record systems for documentation and patient care.
