RN-Home Health Part Time

The RNField Clinician position will provide nursing care which includes: comprehensive health and psychosocial evaluation monitoring of patient's condition health promotion and prevention coordination of services promotion of patient/family independence Responsibilities Provide or supervise skilled nursing services in accordance with physician's orders Asses and regularly re-evaluate the nursing needs of the patient Develop, initiate, implement, and update the plan of care as needed or at lease every 60 days Initiate preventative and rehabilitative nursing procedures as appropriate for the patient's care and safety Prepare clinical notes and OASISassessments Coordinate services and inform physician and other personnel of changes in the patient's condition and needs Observe and report patient's response to treatment and any changes in his/her condition to the physician and supervising registered nurse Function as the patient advocate in all medical decisions affecting the patient Evaluate the effectiveness and outcomes of care Plan for the patient's discharge from home care services Requirements Must hold a current license issued by the Louisiana State Board of Registered Nurse Examiner's without restrictions Must have a minimum of one year experience as a registered nurse Current CPR certification Critical thinking skills, compassionate Home Health experience is preferred but not required ER or ICU experience is preferred but not required #J-18808-Ljbffr
