Part-Time Licensed Practical Nurse

Located in Medford, NY

Salary- Up To $40/HR (Depends on Experience)

Responsibilities:

• Calls the home care coordinator, RN or supervisor with verbal report of initial findings.
• In conjunction with the family and physician and/or authorized practitioner and Americare Supervisor adheres to the Plan of Care based on the RN's assessment.
• Provides nursing care as outlined in the Plan of Treatment to include the following: Nursing services, treatments and preventive procedures including those requiring substantial specialized skill and ordered by the physician or authorized practitioner.
• Initiates preventive and rehabilitative nursing procedures as appropriate for the patient's care and safety.
• Evaluates the effectiveness of his/her nursing service to the individual and family.
• Observes signs and symptoms and changes in the patient's physical or emotional condition and consults with the physician or authorized practitioner concerning alteration of the plan of treatment in consultation with the supervisor, as necessary.

Qualifications:

• Must have a valid New York LPN license
• Must have home care experience
• Must have prior experience as an LPN
• Excellent verbal and written communication skills.
