At Mattapan, our Registered Dietitians have the opportunity to work directly with residents and care teams to ensure nutrition is an intentional part of each resident's individualized care plan.If you're looking for a role where your clinical expertise can make a meaningful difference in the lives of the people we serve, we'd love to have you join our team.ResponsibilitiesWhat You'll Own as a Registered Dietitian (RD/RDN/CDN): Clinical nutrition and resident careComplete comprehensive nutrition assessments and reassessmentsIdentify residents at risk for malnutrition, unintended weight change, dehydration, impaired wound healing, or other nutrition-related concernsDevelop, implement, monitor, and revise individualized nutrition care plansMake evidence-based recommendations based on diagnoses, laboratory information, medications, intake, weight trends, treatment goals, and resident preferencesMonitor residents receiving enteral nutrition and recommend adjustments as clinically indicatedSupport nutrition planning for residents receiving dialysis or complex wound careProvide nutrition counseling and education to residents and familiesDocument assessments, interventions, recommendations, and outcomes accurately in the electronic medical recordInterdisciplinary collaborationParticipate actively in care-plan meetings, clinical reviews, and interdisciplinary discussionsPartner with nursing and medical providers regarding changes in condition, weight trends, laboratory information, hydration, supplements, and nutrition interventionsCollaborate with rehabilitation and speech-language professionals to support recovery, function, intake, and safe nutrition plansWork with dining services to translate clinical recommendations and resident preferences into appropriate meal serviceServe as an accessible nutrition resource for facility leaders and care teamsQuality, education, and regulatory readinessMonitor nutrition-related quality indicators, trends, and outcomesConduct audits and contribute to performance-improvement initiativesSupport compliance with CMS, New York State Department of Health, and applicable nutrition-services requirementsHelp maintain survey readiness through accurate documentation and consistent clinical practicesProvide nutrition education and in-service training to employeesSupport and mentor dietetic interns or Diet Technicians when applicableQualificationsWhat You'll Need as a Registered Dietitian (RD/RDN/CDN): RequiredBachelor's degree in nutrition, dietetics, or a related fieldCurrent Registered Dietitian or Registered Dietitian Nutritionist credential through the Commission on Dietetic RegistrationNew York State Certified Dietitian-Nutritionist credential or other applicable state credentialStrong clinical-assessment, critical-thinking, documentation, counseling, and communication skillsAbility to collaborate effectively with residents, families, nursing employees, medical providers, rehabilitation professionals, and dining servicesAbility to use electronic medical records and applicable nutrition softwareAbility to read, write, speak, and understand English at the level necessary for safe care and effective workplace communicationPreferredClinical nutrition experience in skilled nursing, rehabilitation, long-term care, geriatrics, dialysis, acute care, or another medically complex settingExperience with enteral nutrition, renal considerations, wound healing, unintended weight change, or malnutritionFamiliarity with CMS and New York State nutrition-services requirementsRegistered Dietitians coming from hospitals, dialysis, rehabilitation, outpatient clinical care, or other healthcare settings are encouraged to apply.
