Hospice Registered Nurse Case Manager

Job Description We are seeking an experienced and compassionate Registered Nurse (RN) Case Manager to join our Hospice team. This role is responsible for managing a patient caseload of 12-15, coordinating interdisciplinary care, and ensuring comfort and dignity for patients and end-of-life. Responsibilities Manage and coordinate care for a caseload of hospice patients in Oakland County Perform comprehensive nursing assessments and develop individualized care plans Collaborate with physicians, social workers, aides, and chaplains Provide education and emotional support to patients and families Ensure compliance with Medicare hospice regulations and documentation standards Participate in education groups at facilities Qualifications Active RN license in the State of Michigan Minimum 1 year of hospice or home health experience preferred Strong clinical assessment and critical thinking skills Excellent communication and compassion for end-of-life care Reliable transportation, valid driver’s license and car insurance Benefits 401(k) Competitive salary Dental insurance Free uniforms Health insurance Life insurance Mileage reimbursement Paid time off Vision insurance Training & development License/Certification RN License (Required) Willingness to travel 75% (Required) Work Location Hybrid remote in Bingham Farms, MI 48025 #J-18808-Ljbffr
