Licensed Practical Nurse

Major duties as assigned, but not limited to:

• The incumbent observes patients, documents care and responses accurately and promptly reports changes in condition requiring higher-level intervention. Provides assigned nursing care in Primary Care Clinics, including collecting and documenting patient data, completing clinical reminders, administering immunizations per protocol, assisting with minor procedures, reinforcing patient and family education, and performing delegated treatments such as EKGs, dressing changes, ear irrigations, nail care, and specimen collection. Performs phlebotomy and independently collects and processes a variety of laboratory specimens following safety and labeling standards.
• Supports care coordination by completing pre-appointment calls, addressing Message Manager alerts, communicating patient needs to the team, documenting transfers, and warm handoffs, answering phones courteously, gathering outside medical information, and assisting PACT staff as needed. Measures and records vital signs, observes and reports physical or behavioral changes, and provides basic health teaching following RN guidance.
• Maintains clinic readiness by restocking supplies, checking equipment (AEDs, refrigerators, monitoring logs), monitoring expiration dates, cleaning and storing RME equipment per policy, and reporting malfunctions. Ensures safe and timely responses in emergencies.
• Delivers patient-centered, population-focused care by fostering proactive, personalized health promotion, supporting prevention services, and engaging collaboratively with team members to improve processes and outcomes. Demonstrates professionalism, excellent customer service, effective communication, flexibility, and conflict-resolution skills to maintain strong rapport with patients and staff.
• Completes mandatory training, adheres to policies and standards, and upholds a culture of excellence, safety, and teamwork within the clinic environment.

Work Schedule: Monday, Wednesday, Thursday and Friday (Tuesday Off), 7:00 am - 5:30 pm

Pay: Competitive salary and regular salary increases. When setting pay, a higher step rate of the appropriate grade may be determined after consideration of higher or unique qualifications or special needs of the VA (Above Minimum Rate of the Grade).

Paid Time Off: 37-50 days of annual paid time offer per year (13-26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year) Selected applicants may qualify for credit toward annual leave accrual, based on prior work experience or military service experience.

Parental Leave: After 12 months of employment, up to 12 weeks of paid parental leave in connection with the birth, adoption, or foster care placement of a child.

Child Care Subsidy: After 60 days of employment, full time employees with a total family income below $144,000 may be eligible for a childcare subsidy up to 25% of total eligible childcare costs for eligible children up to the monthly maximum of $416.66

Retirement: Traditional federal pension (5 years vesting) and federal 401K with up to 5% in contributions by VA
