Role: Physician Assistant responsible for conducting comprehensive in-home health evaluations for vulnerable populations. Key Responsibilities Obtain patient histories, perform focused physical exams, review medications, and document findings in an EMR system. Support value-based care by identifying care gaps and reinforcing treatment plans. Coordinate follow-up care and collaborate with interdisciplinary teams. Utilize clinical and analytics tools to deliver patient-centered, culturally sensitive care. Maintain high-quality documentation and ensure productivity standards.
