About the Position

Key Responsibilities

• Perform initial in-home pre-evaluations of patients, including assessment of health status, strengths, care needs, and preferences to guide the development of individualized long-term care service plans
• Review consumer medical documentation and health referral forms relevant to each case and enter evaluation data into electronic evaluation forms
• Document concerns, conflicting information, and issues that surface during the evaluation process and record case notes into a CRM system
• Facilitate communication between the call center and eligible consumers for enrollment into a managed care health plan, and discuss healthcare options, supports, service vendor options, and waiver options with consumers
• Enhance collaborative relationships with interdisciplinary care team members to improve care coordination and facilitate service delivery
• Identify opportunities for health promotion and illness prevention while maintaining a comprehensive working knowledge of community resources, payor requirements, and network services for the target population

• Current Registered Nurse (RN) licensure in New York State
• BLS certification required prior to start date
• Minimum of two (2) years of experience in a home setting within the last fifteen (15) years
• Recent experience conducting comprehensive assessments of patients
• Ability to work with technology in the workplace, including electronic evaluation and CRM systems
• Candidates may not simultaneously conduct UAS assessment assignments with other organizations and will be required to sign a Conflict of Interest (COI) form prior to start

• Bilingual Spanish-English proficiency strongly preferred
• Experience working with multi-generational and diverse consumer populations with sensitivity to individual concerns
• Familiarity with UAS assessment criteria and long-term care enrollment processes
• Ability to manage multiple complex tasks simultaneously in a fast-paced, deadline-oriented environment
