As a Direct Support Professional (DSP) in our Community-Based Day Supports (CBDS) program, you will provide hands‑on, person‑centered support that helps participants build life skills, foster independence, and actively engage in their communities. What You’ll Do Support individuals in daily activities that promote independence, skill development, and community inclusion. Assist participants in achieving goals outlined in their Individualized Service Plans (ISPs). Provide guidance and encouragement in developing communication, social, self‑advocacy, and independent living skills. Accompany participants on community outings and facilitate participation in recreational, volunteer, educational, and vocational activities. Safely transport participants to and from the program site and community activities using agency vehicles or, when necessary, a personal vehicle in accordance with agency policy. Provide personal care assistance, including mobility, hygiene, and activities of daily living, as needed. Monitor participant health, behavior, and safety, reporting concerns or incidents promptly to the Program Team Lead. Complete accurate and timely documentation, including attendance records, daily progress notes, and incident reports. Collaborate with the Program Team Lead and interdisciplinary team members to provide consistent, high‑quality services. Maintain professional communication with participants, families, guardians, and community partners. Participate in staff meetings, required trainings, and ongoing professional development. Perform other duties as assigned to support program operations. Qualifications Required Qualifications High school diploma or equivalent. Experience supporting individuals with developmental or intellectual disabilities or in a related human services role preferred. Strong interpersonal and communication skills. Ability to obtain and maintain CPR/First Aid and SafetyCare certifications. Valid driver's license and reliable transportation. Preferred Qualifications Associate's degree in Human Services or a related field. Experience with Community‑Based Day Supports (CBDS) or adult day services. Working Conditions & Physical Requirements Ability to stand, walk, bend, reach, and lift up to 25 pounds. Provide physical assistance to participants as needed, including mobility support and SafetyCare interventions. Safely transport participants and travel throughout the community. Work in both indoor and outdoor settings in varying weather conditions. Use computers and mobile devices to complete required documentation. Adapt to changing participant needs while maintaining a safe and professional environment.
