Social Service Director

We're seeking a Social Service Director to lead our awesome social services department.  This is an opportunity for someone to join a cool team and a great bunch of people. Work flexibility, benefits, and vacation time. 

What You’ll Do:

·                Support residents and families during transitions and throughout care

·                Documentation, assessments, and care plans

·                Discharge planning

·                Collaborate with the care team and external agencies

·                Be the go-to for social needs, referrals, and resources

What You Bring:

·                LPN, Associates, Bachelor’s 

·                Long-term care and supervisory experience (preferred)

·                Strong communication, empathy, and organization skills

·                Ability to work independently and productively
