Ambulatory Social Worker II Care Coordination

UNC Health in North Carolina is expanding social work services within the Wound Healing, Podiatry, and Vascular programs to improve access, care coordination, and psychosocial support for patients with complex medical needs. As a Social Worker – Ambulatory II, you will conduct assessments, develop person-centered care plans, facilitate resources, advocate for patients, and monitor progress as part of an interdisciplinary team.
