Evergreen Healthcare Group is seeking a Clinical Liaison in the Tacoma, WA area to bridge hospitals, families, and two skilled nursing facilities (Alaska Gardens and Heartwood Extended Care). The role focuses on patient transitions to post-acute care, building trust with partners, and ensuring smooth admissions.
