Physical Therapist - Sign on Availability Primary Location : Munson Med Center - Elk Rapids, MI Employment Type : Full time - Days Compensation : $76,003 - $110,198 Sign on Bonus : $5,000 Days Worked : Monday - Friday Other Benefits : Annual Merit Increases, Health Insurance (Medical, Dental, Vision), PTO, Holiday Pay, PSLF Eligible, Tuition Reimbursement Responsibilities Assumes full responsibility for evaluating patient’s need for physical therapy, planning and executing individual treatment programs in accordance with the State of Michigan licensure and PT scope of practice. Provides patient care, treatment, and services within the scope of license, certification or registration and as required by law and regulation. Evaluates patient condition; establishes treatment goals in collaboration with patient and family, establishes written treatment plan, and delegates responsibilities to PT assistants and rehab technicians as appropriate. Re‑evaluates patient status and treatment effects, revises treatment plan and reviews revised goals with patient and family as appropriate. Evaluates patient’s need for medical equipment, orders appropriate equipment, and instructs on its use. Participates in planning for patient discharge and communicates with patient, family, community support/services and environmental limitations. Demonstrates competence in relevant software and hardware required to meet job responsibilities. Documents patient status and progress through evaluation, progress notes, and discharge summary per internal/external time standards and reimbursement guidelines. Communicates with interdisciplinary team to integrate PT treatment, participates in conferences/discussions, and instructs patient and family in procedures and education. Attends various meetings and in‑services as designated. Follows all hospital policy and procedures including patient confidentiality and infection control. Meets or exceeds program productivity expectations. Provides discipline‑specific coverage (cross training) within the Mary Free Bed system as requested. Adheres to Mary Free Bed Standards of Behavior when working with patients, coworkers and community members. Supports program operations as assigned. May perform additional duties to meet program or hospital needs: orientation and training of new staff, serve as clinical instructor for PT students (supervise and evaluate skills), participate in program development and research activities, meet or exceed clinical outcome standards and performance goals. Age Specific Responsibility Demonstrates knowledge and skills necessary to provide care appropriate to the age of the patients served, knowing principles of growth and development over the life span, assessing data reflective of patient status and interpreting the appropriate information to identify age‑specific needs. Qualifications Graduate of Commission on Accreditation in Physical Therapy Education (CAPTE). Licensed in all US states and territories where practicing, meeting each state’s requirement.
