Community Health Educator Screening RN

About This Role The Community Health Educator Screening RN at Hackensack Meridian Health is responsible for participating in health fairs and community events to provide health resources, education, and clinical screenings. This role is ideal for individuals who are passionate about community health and education. Highlights Location: Maywood, NJ Schedule: Per Diem What You'll Do Coordinate and conduct community health education programs on various topics. Conduct health screenings such as blood pressure, cholesterol, and glucose screenings. Transport, assemble, and break down screening equipment and materials for events. Provide education to community members on screening results and referrals for follow-up care. Accurately collect, input, and maintain data in the outreach database. Obtain signed consent forms for screenings and ensure their safekeeping. Oversee per diem staff and/or medical students assisting with screenings. Maintain an adequate inventory of medical and general supplies for outreach activities. Respond to inquiries from the call center and community members regarding services. Identify and maintain relationships with community organizations as directed. Requirements Education: Associate's in Nursing required Bachelor of Science in Nursing (BSN) preferred Experience: Experience in community health education or a similar role preferred Licenses & Certifications: NJ State Professional Registered Nurse License required Valid Driver's License from a USA state required BLS AHA Basic Health Care Life Support HCP Certification required About Hackensack Meridian Health Hackensack Meridian Health is a leading healthcare provider committed to delivering exceptional patient care.
