Ambulatory Care Pharmacist - Part Time 32 Hours - Fontana Job Type: Part-time Location: Fontana, CA (with coverage at Ontario Oncology Pharmacy; working at Fontana and Ontario Oncology Pharmacy) Responsibilities Provide ambulatory pharmaceutical care services using approved protocols in collaboration with other health professionals, including managing patient drug therapy and providing patient education. Promote high quality, evidence-based, cost-effective pharmaceutical care in clinical areas assigned. Uphold Kaiser Permanente's Policies and Procedures, Principles of Responsibilities and applicable laws. Conduct patient medication history inquiries and synthesize treatment plans based on diagnoses, including drug therapy and lifestyle modifications. Identify, resolve, and prevent potential and actual medication-related problems (e.g., untreated indications, improper drug choice, subtherapeutic dosage, overdose, ADRs, drug interactions). Under protocol, initiate, adjust, monitor, and recommend changes in drug therapy based on patient interviews, monitoring parameters, drug profiles and medical records. Coordinate and interpret laboratory monitoring of drug therapy (serum drug levels, electrolytes, and other approved tests). Provide patient monitoring to evaluate and optimize outcomes, drug utilization, patient compliance, and to minimize ADRs and unnecessary drug use. Educate patients about disease states, expected actions, side effects, proper compliance, and any special instructions. Ensure ongoing performance improvement of pharmaceutical care services provided. Educate providers and staff through presentations, newsletters, utilization reviews, and one-on-one interactions. Perform drug utilization review, comprehensive medication review, and/or medication reconciliation. Operate pharmacy computer systems and other equipment; document information in patient records according to policies and regulatory requirements. Support a collaborative Labor-Management Partnership environment. Oversee ancillary/support staff (technicians, assistants, coordinators, interns). Performs other duties as required. Experience and Qualifications Minimum twelve hundred (1200) hours experience in the last five years under collaborative drug therapy management protocol (initiating, adjusting, discontinuing, and refilling medication for chronic diseases) or successful completion of an ASHP Accredited PGY-1 residency. One of the following credentials or experiences: Board Certified Oncology Pharmacist (BCOP); completion of PGY2 in oncology practice; 1000 hours in sterile compounding oncology pharmacy with clinical protocol management and achieving BCOP within three years; ASHP-accredited PGY1 Residency with oncology rotation and BCOP within three years. Education: Doctor of Pharmacy (Pharm.D.) or degree meeting minimum licensure requirements. Licenses and Certifications: Pharmacist License (California) required at hire; Basic Life Support within 3 months of hire; National Provider Identifier required at hire. Additional Requirements: Pass competency assessment (except for current Ambulatory Care Pharmacists); broad knowledge of pharmacology, medical terminology, disease state management, and applicable laws/regulations; experience with aseptic techniques and preparation of sterile IV products; ability to extract information for medical management plans; strong teaching, communication, and problem-solving skills; works well in a multidisciplinary team. Preferred Qualifications: Pharm.D.; PGY-2 residency or fellowship; board certification in a relevant practice area; Advanced Practice Pharmacist license.
