Special Programs Case Manager I (Licensed BH professional)

Special Programs Case Manager I – Licensed/Certified Behavioral Health Role Locations: Richmond, VA; Norfolk, VA; Suffolk, VA; Smithfield, VA; Virginia Beach, VA; Roanoke, VA; Lynchburg, VA; Charlottesville, VA, and Chesapeake, VA. Field: This field‑based role enables associates to primarily operate in the field, traveling to client sites or designated locations as required, with occasional office attendance for meetings or training. The approach ensures flexibility, responsiveness to client needs, and direct, hands‑on engagement. Alternate locations may be considered if candidates reside within a commuting distance from an office. Travel: Some travel within your assigned region (facility or home visits) is expected. Schedule: Monday – Friday, 8:00AM – 5:00PM Eastern Time. The Special Programs Case Manager I is responsible for performing case management telephonically and/or by home visits within the scope of licensure for special programs, such as Foster Care. The role manages overall healthcare costs for the designated population via integrated (physical health/behavioral health) case management and whole‑person health. Primary duties may include, but are not limited to: Conduct assessments to identify individual needs. Develop comprehensive care plans to address objectives and goals identified during assessment. Support member access to appropriate, quality, and cost‑effective care and modify plans as needed. Coordinate with internal and external resources to meet identified needs of the member in terms of integrated (physical and behavioral) whole‑person care. Coordinate social determinants of health to meet the needs of the member and incorporate that into care planning. Work closely with various state agencies. Maintain knowledge of the system‑of‑care philosophy; a spectrum of effective, community‑based services and supports for those with or at risk for mental health or other challenges and their families, organized into a coordinated network. Build meaningful partnerships with designated populations and their families, addressing cultural and linguistic needs to help them function better at home, in the community, and throughout life. Evaluate health needs and identify applicable services and resources with members and their families. Provide patient education, medication reconciliation, and identification of community resources and assist with arrangement of follow‑up care. Required Qualifications: MS/MA in social work, counseling, or a related behavioral health field, or a degree in nursing. A minimum of 3 years of clinical experience in social work counseling with a broad range of experience with complex psychiatric and substance abuse or substance‑abuse disorder treatment, or an equivalent combination of education and experience. An active, current, and valid license such as an LCSW, LMSW, LPC, LAPC, LMFT, LMHC, or RN issued by the Commonwealth of Virginia. Preferred Qualifications: Experience working with specialty populations. Case management with a broad range of complex psychiatric/substance abuse and/or medical disorders. Knowledge of the Virginia Foster Care system. Prior experience with the Community Services Board (CSB) and/or Department of Social Services (DSS). Traveling to worksites and other locations when necessary. Computer literacy and experience using Microsoft applications (Word, Excel, Outlook).
