The Clinical Care Manager coordinates care across inpatient to outpatient transitions, working with physicians, nurses, and care teams to ensure timely and effective patient management. Required is a Bachelor’s in Nursing with RN licensure; two years of clinical experience preferred. Masters is a plus.
