Back to Health is growing and expanding! Looking for motivated and energetic PTs to perform home visits that are passionate to help the senior population!

We are looking for experienced, energetic and motivated NJ licensed Physical therapists to perform home visits to the senior population in Franklin Lakes, NJ and the surrounding towns in Bergen and Passaic Counties, NJ.

We have assembled a team of outstanding physical and occupational therapists that are motivated to make a true difference in their patients' lives.

Home Care Responsibilities

• Provide treatments that help improve clients' motion and mobility
• Physical Therapist will be responsible for Evaluating and Treating patients with a variety of orthopedic and neurological disorders.
• Develop care plans using a variety of treatment techniques
• Provide quality, personalized and evidence-based care and proven interventions
• Motivate patients during treatment in order to help them function optimally
• Promote clients' healthy lifestyle by improving strength, flexibility, balance, and coordination
• Consult and practice with other health professionals
• Evaluate effects and monitor and communicate progress
• Document patient care services
• Must be organized and have good communication skills

Home Care Physical Therapist Qualifications

• New Jersey Physical Therapist License
• Experience in Home care and/or outpatient orthopedics
• Proficient in electronic documentation
• Excellent Communication and Interpersonal Skills
• Drive to continually learn and grow
• Current knowledge of treatment practices
• Professional behavior
• Excellent interpersonal communication skills
