Licensed Practical Nurse Patient Transport Rehab

CarePartners is seeking a compassionate and safety-focused Licensed Practical Nurse Patient Transport Rehab to support our Healthcare & Medical Services team. In this role, you will provide direct, hands-on nursing care to rehabilitation patients while coordinating and delivering safe, timely transport between facilities, clinics, and specialized care settings. Working within our network of partnered providers, you will help ensure continuity of care by monitoring patient status before, during, and after transport; administering prescribed medications and treatments within LPN scope; and promptly communicating any changes in condition to the broader care team. You will prepare patients for transport by reviewing orders, confirming care plans, and educating patients and families about what to expect, demonstrating the collaboration, compassion, and patient-centered care that are core to the CarePartners culture. During transport, you will perform ongoing assessments of vital signs and comfort, support mobility needs, manage medical equipment such as oxygen or mobility aids, and maintain meticulous documentation in accordance with clinical and regulatory standards. You will collaborate closely with nurses, rehabilitation therapists, case managers, and transport staff to coordinate schedules and routes that prioritize patient safety and comfort while supporting efficient healthcare service delivery across our network. The Licensed Practical Nurse Patient Transport Rehab will contribute to the improvement of community health by helping patients access the right level of rehabilitative care at the right time, supporting discharge planning, and reinforcing therapy and safety instructions consistent with each patient’s rehabilitation goals. You will follow infection-prevention protocols, adhere to HIPAA and other privacy regulations, and participate in quality improvement initiatives related to patient transport and transitions of care. This position is ideal for a detail-oriented LPN with strong clinical assessment skills, excellent communication, and the ability to work independently in the field while remaining an engaged member of an interdisciplinary team.
