Per Diem - RN, Hospice House (7a - 7p) LifePath Hospice

$45 per hour

Job Profile Summary The Per Diem RN is responsible for assessing and identifying patient/family needs, utilizing the nursing process, coordinating the plan of care with the Interdisciplinary Group (IDG), and providing palliative and supportive care to the patient/family unit. Qualifications Current license as RN in the state where the employee will be working Active BLS for healthcare professionals from the American Heart Association or Red Cross Minimum one (1) year nursing experience; hospice or hospital experience preferred Previous experience working with an EMR/EHR (Electronic Medical/Health Record) system Valid driver’s license and automobile insurance per Company policy (Mobile Driver) Reliable transportation to meet visit schedule Ability to use equipment with visual and auditory mechanisms Effective communication in English (verbal and written) Flexible schedule to meet the needs of Hospice House patients Ability to manage the emotional stress of working with and caring for terminally ill patients and their families Physical access to patient rooms, other areas of the Hospice House/Hospital/ALF, and homes, including navigating stairs and narrow spaces, tolerating heat and lack of air conditioning within any part of the service area Ability to perform the essential functions and physical requirements of the job (including lifting patients or equipment, bending, pushing/pulling, kneeling) with or without reasonable accommodation Employees working at PACE must have certification of completion of Alzheimer’s Disease and Related Dementias Training through the Florida Department of Elder Affairs Some locations may require cross‑training to hospice house or field positions of equal licensure/certification and scope when census calls for flexible staffing Ability to obtain physician orders and complete documentation per Company policy Strong relationships with case managers, physicians, and other professional contacts at facilities Ability to comply with all State, federal and local regulations and Company policies, including fraud and abuse prevention, health and safety, and confidentiality of patients and families Use resources in a fiscally responsible manner and foster professional development and knowledge improvement, especially in electronic media skills Engage in quality assessment and performance improvement activities as appropriate for the position Job Responsibilities Provide and manage direct care to patients and families as part of the Interdisciplinary Group (IDG), incorporating psychosocial, spiritual, cultural, physical, and biological components along with appropriate nursing intervention and follow‑up Coordinate the Plan of Care, ensuring an individualized plan that reflects the patient’s evolving needs Educate patients, families, caregivers, and health professionals about the disease process, decline, palliative interventions, caregiving, dying process, and safety practices Report changes in the patient’s condition to appropriate IDG members or other health professionals Participate with the IDG to evaluate hospice referrals or admissions for level of care appropriateness Attend patient deaths, prepare bodies for transport, and oversee disposal of medications Provide bereavement interventions to family and caregivers in collaboration with psychosocial staff Present concise and pertinent oral and written reports to the IDG, encouraging input from all disciplines Communicate accurately and completely with physicians, staff, patients, families, and supervisors, using positive approaches when working with others Supervise patient care given by other nursing team members (LPNs, Hospice Aides, and contract nurses) During emergencies (e.g., hurricanes), report to work at a designated location to ensure continuity of services, possibly before scheduled time or overnight based on emergency duration Complete clinical admissions for all Company services, including explaining services, addressing patient fears, presenting services empathetically, identifying objections, educating on admission requirements, completing informed consents, initiating the Plan of Care, initiating skilled nursing interventions, obtaining physician orders, and documenting per policy Act as the Company representative at assigned facilities, facilitate referrals to all service lines, and maintain communication with Admissions for timely follow‑up Provide care for GIP patients in hospitals, including daily visits, professional management, documentation, and efficient discharge Obtain consents for services, arrange safe discharge, and meet productivity expectations while ensuring timely discharge of referred and eligible patients Obtain medical record information for referrals and maintain strong relationships with case managers, physicians, and PRRs for service education Perform additional duties as assigned Physical Demands Required abilities: see, hear, talk, walk, use hands to finger, handle, or feel Frequently required: stand, sit, reach with hands/arms, lift, bend, balance Occasionally required: pull, push, stoop or crouch, kneel, climb stairs Compensation Pay Rate: $45.00 per hour. Compliance This position requires consent to drug and/or alcohol testing after a conditional offer of employment, as well as ongoing compliance with the Drug‑Free Workplace Policy. All Chapters Health System employees performing services for Florida affiliates are submitted through the Florida Care Provider Background Screening Clearinghouse to verify eligibility after a conditional offer of employment and for ongoing eligibility.
