Discharge & Care Coordination Specialist
Centerstone Medical Group
Centerstone Medical Group in Springfield, Missouri, seeks a Discharge Planner to coordinate discharge planning from admission to post-discharge follow-up. The ideal candidate will develop individualized plans reflecting clinical needs, maintain relationships with service lines, and advocate for clients overcoming care barriers. A Bachelor’s degree in Social Work or Psychology is required, with a preferred Master’s degree.
