• Monday - Friday, no holidays or weekends -- 40 hours/week guaranteed
• Average of 5 - 6 patients per day
• Home health setting -- adult caseload
• Rotating across 4 - 8 different facilities
• Home health and OASIS experience required
• HCHB experience preferred
• Strong communication and documentation skills required
