Community Grant Coordinator

This role coordinates and supports community grantmaking, community investment activities, and related programmatic initiatives across the health system. Responsibilities include serving as the primary point of contact for community grant applicants and grantees; ensuring adherence to grantmaking processes, compliance, and reporting requirements; and supporting the tracking, evaluation, and documentation of community investments. Provides coordination and project support for community health initiatives, including committee facilitation, deliverable development, and project management support.

Primary Responsibilities

• Serve as the primary point of contact for potential applicants and funded partners, providing guidance, technical assistance, and support throughout the full grant lifecycle.
• Coordinate and administer all aspects of community grantmaking processes, including application intake, eligibility screening, review coordination, approval workflows, and applicant communications.
• Track, monitor, and document grant deliverables, outcome measures, and compliance with mid-year and year-end reporting requirements.
• Support grantee relationships through regular check-ins, site visits, and ongoing communication to ensure alignment with grant objectives and organizational priorities.
• Coordinate community investment documentation, agreements, and related administrative tasks, ensuring accuracy, completeness, and timeliness.
• Maintain accurate and up-to-date records within grants management systems, databases, and shared tracking tools to support transparency, compliance, and reporting.
• Prepare reports, summaries, presentations, and dashboards that communicate community grantmaking and investment activity, outcomes, and impact to leadership and internal stakeholders.
• Analyze grant and investment data to identify trends, insights, and opportunities for continuous improvement in grantmaking and community investment practices.
• Collaborate with internal partners (including finance, legal, compliance, and clinical teams) to support grant agreements, payments, reporting, and audit readiness.
• Provide coordination, planning, and project management support for Community Health and Equity in Patient Care initiatives, including committee facilitation, meeting coordination, and development of project deliverables.
• Support the integration of health equity principles into community grantmaking and investment activities, including data collection, evaluation, and partner engagement practices.
• Assist with the documentation, refinement, and implementation of grantmaking and community investment policies, procedures, and workflows.
• Perform all other duties as assigned.

Education & Experience - Required

Bachelor's degree in related field (e.g. Public Health, Health Administration, Business Administration, Nonprofit Management).

Two (2) years previous experience in grant administration, community-based programs, nonprofit organizations, public health, or a related area.

Education & Experience - Preferred

Master's degree preferred.

Four (4) years previous experience in grant administration, community-based programs, nonprofit organizations, public health, or a related area.

Previous experience coordinating grants, contracts, or funded initiatives, including tracking deliverables and reporting requirements.

Previous experience in healthcare, public health, or nonprofit sector.
