RN Admissions Nurse

Bishop Rehabilitation & Nursing Center is looking for a RN Admissions Nurse to work in our facility in  Syracuse, NY .

11AM-7PM

Duties & Responsibilities:

• Complete admission assessments for new residents and hospital returns, including starting the initial plan of care.

• Help support and guide the Nursing Department as directed by the Director of Nursing.

• Review PRI information and work with the interdisciplinary team to help coordinate the admission process.

• Review resident records and complete audits to make sure facility standards and policies are being followed.

• Review and update resident care plans when there are changes in the resident’s condition.

• Assist with discharge planning as needed.

• Complete all required documentation and charting in a timely manner.

• Complete MDS assessments, care plans, and other required paperwork as assigned.

• Help oversee and direct resident care when needed.

• Maintain resident privacy and follow all HIPAA and company confidentiality guidelines.

Requirements & Qualifications:

• Must have an active and unrestricted NYS Registered Nurse (RN) license.

• Previous experience working as an Admissions Nurse in a long-term care setting.

• Strong clinical assessment and critical-thinking skills.

• Able to make sound decisions and work independently.

• Strong leadership and supervisory skills.

• Comfortable using computers and Microsoft Office programs, including Word, Excel, and Outlook.

• Long-term care experience is preferred.

• Strong communication and people skills.
