Hospice RN Case Manager (PRN) - Houston Region

Pathways Hospice in Texas is seeking an experienced RN Case Manager to coordinate care for patients and families. This PRN role covers a territory including Northwest, Southwest, and Southeast Houston, with on-call rotations and weekend coverage as needed. You will supervise nursing care, collaborate with physicians and therapists, plan home care services, assess patient needs, educate families, and maintain complete clinical notes.
