Schedule

Monday to Friday, 8:00 AM to 5:00 PM, with on-call and weekend rotations as needed.

Our Hospice Nurses are not only skilled clinical practitioners but also educators, guiding patients in identifying their personal care goals while helping caregivers gain confidence in their roles. As a generalist in all diseases and a specialist in symptom management, you will demonstrate excellence in assessment and critical thinking. You will lead an interdisciplinary team to provide the best possible care, serving patients in various settings, including private homes, nursing homes, group homes, or assisted living facilities.

Responsibilities

• Manage an assigned caseload of patients and coordinate with the care team.
• Deliver comprehensive and compassionate nursing care in diverse environments.
• Educate patients and families about care goals and symptom management.
• Conduct thorough assessments and develop care plans tailored to individual patient needs.

Required Experience / Certifications / Licensure

• Valid RN license in the state of practice.
• Experience in hospice or palliative care is preferred, but not required.
• Strong clinical assessment and critical thinking skills.
