Job Type: Full-time Projected Total Compensation: $115,000 - $165,000 per year Benefits: • 401(k) matching • Dental insurance • Employee discount • Health insurance • Paid time off • Vision insurance Ability to Commute: • Minneapolis, MN 55447 (Required) Work Location: In person Responsibilities: • Perform patient assessments, physical exams, and chart reviews longitudinally. • Order and interpret labs/diagnostics; develop and adjust treatment plans. • Diagnose, manage, and treat acute and chronic conditions. • Prescribe and manage medications per state and federal guidelines. • Provide patient, family, and staff education on health and prevention. • Collaborate with Aeris team members and facility staff to ensure seamless care. • Document accurately and support quality improvement initiatives. Qualifications: • Current Nurse Practitioner license in Minnesota. • Board Certified Nurse Practitioner or Clinical Nurse Specialist • 1+ year of experience in a skilled nursing facility or similar setting preferred. • Strong background in geriatric, primary, acute, and chronic care. • Excellent communication, teamwork, and problem-solving skills. • Commitment to patient-centered care, ethics, and continuous learning. Compensation: $116,000 - $165,000
