We seek individuals who are driven to make a difference and embody our motto, “To Care is an Honor.” Position Experienced, Per Diem, Hospice Social Worker for the Kansas City, KS area. Responsibilities Conduct a psychosocial assessment within 5 days of admission to the hospice program, identifying appropriate psychosocial problems, interventions and goals for the patient’s plan of care. Participate in IDG meetings and develop the patient’s plan of care. Observe, record and report changes to the patient’s emotional and social factors that are being affected by the illness and need for care. Maintain and submit written clinical records as required, including the initial evaluation, care plan and daily notes. Evaluate patients and families’ responses to and effectiveness of medical social work interventions. Confirm weekly scheduling of visits with the patient’s Case Managers to coordinate other staff visits. Participate in IDG and QAPI meetings and activities. Continue to offer support to the IDG if the patient/family declines Social Work services, monitoring evolving needs. Assume responsibility for self-development by learning through formal education, workshops, conferences, professional meetings, and/or research. Assess patient and family members’ ability to cope with the patient’s dying. Act as consultant to health team members, assisting them in understanding social, emotional and environmental factors related to the patient’s health problems. Help patients utilize resources of their families and the community. Qualifications Master’s in Social Work (LMSW) required. Hospice experience preferred. Ability to reliably handle and maintain confidential information. Must be a licensed driver with an insured automobile in good working order. Strong verbal and written communication skills. Must disclose any current or future need for employment-based immigration sponsorship before an offer of employment is extended. Pay Rate $40.00 per visit, $65.00 per admission and mileage reimbursement at $0.55 per mile.
