Position: Primary RN - Case Manager (Community Hospice)
Facility: HopeWest-Hospice
Location: Grand Junction, CO 81506
Job Type: Contract
Contract Length: 6 Weeks
Shift: Monday-Friday | 8:00 AM - 5:00 PM
Guaranteed Hours: No Guarantee
Pay Range: $55-$58/hr

Requirements:

Active Colorado RN License
Minimum 2 years of RN experience
Ability to work independently and collaborate with an interdisciplinary team

Responsibilities:

Manage an assigned hospice patient caseload.
Coordinate and provide compassionate end-of-life care in patients' homes and care facilities.
Develop and update individualized care plans.
Educate patients and caregivers on symptom management and treatment plans.
Collaborate with physicians and the interdisciplinary care team to ensure high-quality patient care.
