- Pay Range: $55-$58/hr
- Active Colorado RN License
- Minimum 2 years of RN experience
- Ability to work independently and collaborate with an interdisciplinary team
- Manage an assigned hospice patient caseload.
- Coordinate and provide compassionate end-of-life care in patients' homes and care facilities.
- Develop and update individualized care plans.
- Educate patients and caregivers on symptom management and treatment plans.
- Collaborate with physicians and the interdisciplinary care team to ensure high-quality patient care.
