Job Title: Registered Nurse (RN) - Hospice Admissions Job Type: Full-Time Location: Denton County / Dallas-Fort Worth (DFW) Metroplex, Texas Work Setting: Community-Based / Home Hospice Schedule Full-Time Monday - Friday
9:00 AM - 6:00 PM
Summary We are seeking a compassionate and experienced Registered Nurse (RN) to join our team as a Hospice Admissions Nurse. As the first clinical point of contact for patients and families, you will play a vital role in providing compassionate guidance through the hospice admission process while ensuring accurate eligibility assessments and seamless care coordination. This position is ideal for an RN with hospice experience who thrives in an independent, patient-centered environment and is comfortable traveling throughout the DFW Metroplex. Responsibilities Evaluate and assess patients to determine hospice eligibility. Guide patients and families through the hospice admission process with compassion and professionalism. Develop and initiate individualized plans of care. Collaborate with Clinical Managers and the interdisciplinary team to ensure smooth patient onboarding. Complete accurate and timely clinical documentation. Participate in limited weekend on-call phone triage rotation. Qualifications Required Current, active Texas Registered Nurse (RN) license. Minimum of 1 year of recent hospice experience. Prior hospice or senior care experience. Thorough understanding of hospice philosophy, eligibility criteria, and Medicare hospice benefits. Reliable transportation, valid driver's license, and current auto insurance. Successful completion of background check and professional reference verification. Preferred Hospice admissions experience. Experience using Consolo EMR.
