Care Plan Facilitator (RN) | 40 Hours per Week | Klein Center page is loaded## Care Plan Facilitator (RN) | 40 Hours per Week | Klein Centerlocations: West Burlington, Iowatime type: Full timeposted on: Posted 2 Days Agojob requisition id: R-08536**Minimum Hiring Wage:**From $74,443.20 a year**Job Details:**Scheduled Hours: 8:00am - 4:30pm**Job Description:****A Brief Overview**Coordinates data collection for the Resident Assessment Instrument (RAI), including the MDS, CAAS and the Multidisciplinary Care Plan for long term care.**What you will do*** Completion of the MDS, CAAS and Care Plans are done by required completion dates and submit to the state of Iowa by submission date.* Evaluate nursing and clinical intervention outcomes, conferring with other health care team members as necessary.* Monitor resident medication usage and results.* Participates in discharge planning and other necessary team conferences.* Interview resident as designated and review documentation related to medical conditions, such as adverse reactions to medication or treatment, condition change, or progress or lack of progress with interventions.* Work as part of a health care team to assess resident needs, plan and modify care, and implement interventions as appropriate.* Demonstrates accurate keyboarding proficiency and computer navigation skills.* Participate in activities aimed at professional growth and development including conferences or continuing education activities.* Collaborates with assigned RN to refer residents requiring more specialized or complex treatment to psychiatrists, primary care physicians, or other medical specialists.* Must have current knowledge of the entire RAI process and current regulatory requirements related to this process and implement.**Qualifications*** Associate's Degree (AA) In nursing (ADN) Required* Bachelor's Degree in Arts/Sciences (BA/BS) In nursing (BSN) Preferred* 1-3 years Nursing home facility experience within the last three years Required* 1-3 years Experience in the RAI process Preferred* 1-3 years Experience with Microsoft Suite (Word, Excel, PowerPoint, Teams, and Outlook) Preferred* Knowledge of principles and processes for providing customer and personal services.
