Registered Nurse - Case Management - Hospital

Job Description

Job Summary:
The case management registered nurse is responsible for coordinating discharge planning and making appropriate referrals for inpatient clients. They work closely with interdisciplinary teams to ensure effective patient transitions from hospital to home, community, or skilled nursing facilities.

Responsibilities:

• Collaborate with physicians, social workers, and other healthcare team members to develop and implement discharge plans.
• Assess patient needs and facilitate referrals for housing, community resources, and specialized services such as wound care and dialysis.
• Educate patients and families regarding post-discharge care and ensure all documentation is complete.
• Document patient progress and discharge plans accurately within the EMR system.
• Handle a caseload of 17-25 patients, including 33-40 patient floors, often working independently on complex cases.
• Participate in unit rounding and interdisciplinary meetings to ensure seamless patient care.

Qualifications:

• Possess at least 1 year of inpatient case management experience.
• Hold current BLS certification and PRI & SCREEN certifications (can be obtained online).
• Demonstrate knowledge of discharge planning, referral processes, and external tools such as CarePort, MCG, or InterQual.
• Be able to work independently and efficiently manage a caseload of at least 20 patients.
• Exhibit strong critical thinking, time management, and positive attitude.
• Experience with EMR systems, preferably Allscripts.
