Part-Time Physical Therapist

Happy at Home Consulting is hiring a part‑time Physical Therapist to provide in‑home therapy services for older adults across the Des Moines metro and surrounding communities. We are looking for a PT who enjoys working with older adults, values practical problem‑solving in the home environment, and is comfortable providing care that supports independence, mobility, fall risk reduction, caregiver education, and aging well at home. This role is a good fit for someone who wants meaningful clinical work without the pace or structure of a traditional facility setting. Our visits are completed in the client’s home, where recommendations can be realistic, individualized, and immediately connected to daily routines. Position Title: Part‑Time Physical Therapist Employment Type: Part‑time Setting: In‑home outpatient therapy, community‑based programs, telehealth when appropriate Location: Des Moines metro and surrounding communities Schedule: Flexible daytime schedule, no routine nights or weekends Reports To: HAHC leadership and Clinical Lead as applicable Responsibilities In‑home PT evaluations and treatment visits Fall risk assessment and intervention Mobility, balance, strengthening, and functional training Home exercise instruction and progression Caregiver education and support Recommendations for equipment, home setup, and daily routines Documentation in HAHC‑approved systems Care coordination with clients, caregivers, referral partners, and other providers Telehealth services when clinically appropriate and payer‑approved Participation in required team meetings, training, and quality review activities We are looking for someone who Enjoys working with older adults and caregivers Is comfortable working independently in home and community settings Communicates clearly and professionally Uses good clinical judgment and practical problem‑solving Can manage documentation and scheduling expectations Understands the importance of client‑centered care Is comfortable collaborating with OT, SLP, administrative staff, and community partners Values flexibility, autonomy, and high‑quality care Requirements Active Iowa Physical Therapy license Current BLS/CPR certification or ability to obtain before client care Iowa mandatory reporter training for dependent adult and child abuse Valid driver’s license and auto insurance for home visits Ability to complete required background screening, credentialing, employment physical/work clearance, and TB screening Ability to use electronic documentation and HAHC‑approved communication systems Prior experience with older adults, fall prevention, home health, outpatient, geriatrics, or community‑based care preferred Compensation and benefits Compensation will be based on role, experience, schedule, and service assignment.
