Remote Health & Human Services Case Manager II

The State of Delaware's Department of Health and Social Services (DHSS) seeks a Health/Human Service Case Manager II to determine initial and ongoing eligibility for SNAP, Medical Assistance, CHIP, and other needs-based benefits. You will coordinate with TANF and SNAP providers and deliver case management and counseling to clients. You will work in a multidisciplinary team using a trauma-informed approach to identify barriers, connect vulnerable residents with benefits, and maintain accurate #J-18808-Ljbffr State of Delaware
